Random TPK?

Following a combat encounter, the party interrogates the two surviving guards for information. A party member decides to kill off one of the guards while another gets the last of the information from the surviving guard. After writing a respond to that event, which ends with the NPC stating the party has everything they need for the next part, Franz drops the HP of both players and the NPC to 0 and reports a Game Over without any explanation. I had to undo the 3 events where it dropped the HP in order to continue the campaign.
