Player Character being deleted from my campaign.

Player characters I created from the Character Select have mysteriously been deleted in some of my campaigns. Can't access their character sheet, nor they show up in Party or World, Mentions. They are gone permanently.

The character in this campaign was a human fighter named Ifan. All they are is a undefined avatar with no name.
