NPC replaced randomly with new NPC of same name.

Tavia Windharp started from session 1 as a lvl 3 Female Half-Elf Ranger. and for some reason out of nowhere, there was a prompt for character generation and she was replaced with a random lvl 5 Human Delver.

DM Cannot recall my NPC from 5 minutes ago and is stuck on errors.
