Combat failing

I'm not even gonna provide the ids because it's just happening in every campaign now, dealing damage without attack rolls or saving throws, dealing damage instead of healing, monsters doing nothing sometimes, applying status effects without a saving throw, rolling damage rolls twice instead of once and applying the sum, refusing to acknowledge character, race, or class feats. And I swear every time the LLM's get updated, it just gets worse.
