mystical "a tall woman with calloused hands"

Quite often in completely different campaigns and with completely different characters, a mystical "a tall woman with calloused hands" or some gnome with a beard/man with burnt skin bursts into the description, which does not correspond at all to the descriptions of the characters and situations. It's weird, spoils the game experience and sometimes quite scary. :)

In this campaign it looks like this: "Sylus, from his armchair, remained a picture of serene amusement, his reddish eyes fixed on the brothers, a tall woman with calloused hands watching their familiar dynamics."
