Location and Nearby NPC wont update, wrong NPC description

I traveled to an area requested by an NPC in the game, but the area is “Uknown area” in the tracker. Also the nearby NPC tracker is empty, even though I am traveling with two NPCes that exist in the NPC pool and I manually added them to my party. Prior to adding them and after they never appeared in nearby NPCes, it seems to be buged. In addition, one of the NPCes was described as:
Grimbold Ironfist a Huge Barbarian by the AI.
However, when I go to their character card, they are described as a dwarf cleric. it is not consistent, the description of the NPC during the game and what is written in their character sheet, I think the matching between the two might be bugged as well.
