Please note, I am writing using a translator.

Firstly, I think it would be a good idea to add impassable areas and obstacles to the map so that the character cannot go wherever he wants no matter what.

Secondly, just as I can add specific monsters to the map in places of interest, it would be nice to have a function to put treasures in places where I want them. For example, excalibur in a stone, a hidden box that must be found, or a unique item that will be needed a little later for a quest.

Thirdly, it would be interesting to play on dynamic locations. For example, if you used fifty thousand tons of TNT to win a fight with a dragon, then at least a giant-sized crater should remain on the location. Well, and at least interactive change of locations - this is the ability to take and build a base for yourself anywhere you want and when you return from another foray, you will find your buildings, and not the location in its original form.

Fourthly, the mechanics of equipment wear. Damaged equipment may have reduced stats, and also worsen the results of dice checks. Weapons and equipment could completely break, and on the contrary, be repaired or even improved.

Fifthly, the rational use of certain items. Empty bottles left after using potions, a waterskin that doesn't disappear from your inventory after the first sip, spoiling food.

Sixthly, time. The change of day and night, working hours of shops, curfew, increased danger outside populated areas and from night monsters, as well as the downsides of night megalopolises. This also includes fatigue and the need for rest, food, hygiene of the character.
