Character Sheet Forms & Transformation System

What if we had the ability to use a transformation tab on characters with forms like werewolves / vampires and the like? Something we can set for custom characters that switch the character sheet to an empowered one when used as a skill. Perhaps creation of the forms can work similarly to the monster sheets. Or we could even create a monster sheet and assign it as unique to a specific character so that it would show up in a character's transformation tab. This could come with bonuses and abilities.
